1. Go to "Account Settings."
2. Go to "Payments."
3. Go to "Learn More About Facebook Credits."
4. Go to "Did you pay for an item but didn't receive it in your game?" and click the word "here."
5. Follow instructions at "I was charged by Facebook, but I didn't receive my game item."
